IPRO, National Kindey Foundation to Host Ending Disparities in Chronic Kidney Disease Stakeholder Summit

The IPRO Quality Innovation Network – Quality Improvement Organization has partnered with the National Kidney Foundation through their initiative Ending Disparities in Chronic Kidney Disease Stakeholder Summits. 
 
The stakeholder summits aim to convene 4 working groups, starting in May, of leaders and change-makers in Ohio to collaborate on strategies to improve CKD prevention, diagnosis, and treatment. Below are the main goals for each workgroup you or your team can consider joining:
  1. Clinical Considerations for CKD in Primary Care: To discuss strategies and approaches that can be employed to improve CKD recognition and care in primary care settings.
  2. Engaging Community and Community-Based Solutions: To develop strategies to advance CKD awareness through community engagement and to ensure that health care providers are aware of the community resources available to delay CKD progression.
  3. CKD in a Population Health Model: To develop a strategy to incorporate CKD testing and diagnosis into wellness and prevention practices within the employer/commercial health plans, and the broader community.
  4. Policy, Payment, and Quality Measurement: To develop a strategy to streamline CKD testing in primary care from a policy and payment perspective.
 
The Ending Disparities Summit workgroups will participate in “Learning in Action” meetings to:
  • Analyze the prevalence of undiagnosed chronic kidney disease (CKD) in Ohio, and its impact on patient outcomes, health care costs and health inequities. 
  • Discuss strategies and approaches that can be employed to improve CKD recognition and care in primary care settings.
  • Evaluate the impact the new Kidney Health Evaluation HEDIS measure can have on improving breakdowns in care and develop a strategy to streamline CKD testing in primary care from a policy and payment perspective.
  • Develop a strategy to incorporate CKD testing and diagnosis into community wellness and prevention practices.
  • Develop strategies to advance CKD awareness through community engagement and to ensure that health care providers are aware of the community resources available to delay CKD progression.
